When the form opens, fill it out online.  Form boxes may be checked either by clicking on the box with your mouse, or tabbing to the box and hitting "Enter" on your keyboard.  To fill in a text field, click on that field with your mouse, or tab to the field you wish to fill in.  Then, print the form by selecting the print icon on the Acrobat Reader toolbar.
